A Scored Number Question can be used to collect a numeric response between a minimum and maximum number. The upper limit set in the question settings, will serve as the total points possible for that question so if a scored number is answered 1 and the upper limit is 10 it would score 1/10.
Number Questions and Scored Number Questions work similarly except number questions don't impact the overall or section score while scored numbers score the same amount as the number entered.
To add a Scored Number Question to a form:
Select Scored Menu Question from the Question Format drop down.
Select the position on the form for the question. The default is to add the question to the bottom of the form but it may be positioned after a specific question as well.
Input the Text for the question.
Input the upper and lower limit for the response.
Input the labels for the upper and lower limits. Labels can be displayed in the form only. on the report only or on both.
To change the format of the question select Formatting Questions
To change the question requirements select Question Requirements
To change scoring or add a bonus question select Scoring and Bonus Questions
When all selections are complete, select Add New.
