An easy way to add a question to a survey is from the Client & Form settings page.


Get started at the main Admin Page.


Select a client, head to the Clients tab and use the Clients & Form Settings link in the Setup column, or simply use the Quick Jump bar!



Once on the Client Administration page, find the name of the survey you want to add questions to.



From the Form Settings dropdown, select the Question List option and hit the GO button.



You will be forwarded to a new page, Question List.



The Date shop performed question will automatically be added for you, as that question is required on every survey. 


Add a Question

To add a question, select the question type (1) from the dropdown, add the question position (2) from the second dropdown, and once you're all set use the Add button (3).



Question Options

This will open a new window, allowing you to add the Text of the Question as well as to include the question score in overall score and set it up as a Bonus question, set the requirements for a question, format it and set the question's "N/A" scoring.



The top of the pop-up window will display the Question Order and Question ID the question will have after being added. You can edit the Question Order in this step, if you want to change the question's position.


You will want to input the Text of Question as well, and make sure that the wording matches the question type. For an example, if you are adding a Yes-No question, you want to make sure the answer to your question (text) can be answered with the two offered answers (Yes or No).


The Answer Options section allows you to set the score of the answer, as well as to mark the answer as N/A.


You can use the section selector to assign a question to one, or multiple, sections.


On the topmost section on the right side, you will be able to set the requirements for the question.


The two middle options allow you to include the question in the total score, or mark it as a bonus question.


The bottom section offers an option to set the audience invisibility, level visibility as well as to format the question display to your liking.



Once you are satisfied with your choices, simply hit the Add New button to add your question!



Question Position

When adding questions, or grids, the default setting is to add them to the End of Form


You can, however, use either of the dropdowns to select where your question or grid will be added, based on the Question order number.
